What to Bring to Your First Visit
A little preparation helps your first appointment go smoothly. Please bring the following so we can get to know you and your smile.
- Completed new patient forms, or arrive early to finish them here.
- A photo ID, such as a driver’s license.
- Your dental insurance card, if you have coverage.
- A list of current medications and any known allergies.
- Recent dental records or X-rays, if you are transferring from another office.
- Any questions you would like to ask Dr. Ngo.
Insurance and Payment
We accept most major PPO dental insurance plans and will help you make the most of your benefits. If you do not see your provider below, contact us and we will be happy to check.
No dental insurance? No problem. Ask our team about flexible payment options. We review all costs with you before treatment begins, so there are never any surprises.

Are you accepting new patients?
Yes. We welcome new patients of all ages across Paradise Valley and Scottsdale. Call (480) 445-9898 or request an appointment online to get started.
How early should I arrive for my first appointment?
If your forms are complete, arriving 5 to 10 minutes early is plenty. If you would like to fill them out at the office, please come about 15 minutes early.
What if I do not have dental insurance?
You are still welcome. We accept most major plans and offer flexible payment options, and we always review costs with you before any treatment begins.
How do I complete the new patient forms?
Download the patient packet from this page, print and complete it, and bring it with you. You are also welcome to fill everything out at our office if you prefer.
What should I bring to my first visit?
Bring your completed forms, a photo ID, your insurance card, and a list of current medications. Dental records or X-rays from a previous dentist are helpful too.
